What vaccinations / medical documents do I need?

The vaccinations you need vary from college to college. You will usually receive a medical health form from the college listing the vaccinations. 
In any case, you should keep your immunization record up to date and scan all pages, including the cover page, as a .pdf file.

As a rule, however, you should have the following vaccinations:
  1. Measles, mumps, rubella 
  2. Hepatitis B
  3. Tetanus
  4. Meningitis ACWY vaccination
DDepending on the university, you may need a tuberculosis blood or skin test, and in rare cases, a chest X-ray. 
You may also need to take your medical documents to your family doctor, who will fill them out for you and answer questions about your vaccinations and/or your general health.

Warning
All future NCAA athletes (Division I, II, III) are required to undergo a sickle cell anemia test before they are allowed to participate in athletic activities at their new university.
